Path to MR/DD Services

The Division of Mental Health, Developmental Disabilities and Addictive Diseases serves people of all ages and those with the most severe problems. The MHDDAD regional offices are the contact points for people needing treatment for mental illness or substance abuse problems, support services for people with mental retardation or substance abuse prevention services.

Anyone who needs support services for a person with a developmental disability should contact the Intake and Evaluation Team in their regional office. To find out which regional office your county is in, please click on the link below:

MHDDAD Regional Offices

The Intake and Evaluation Team will complete a screening application based on specific information will provide. After reviewing this information the Intake and Evaluation Team will determine if you or your loved one is eligible for support services under the Division of MHDDAD guidelines.

If you or your loved one is eligible, you will either:

Receive a letter awarding needed support services, at which time a support coordinator will be assigned to assist you in selecting a provider and developing an individual service plan.

Receive a letter stating funding is not available at this time and you have been placed on the planning list for support services. If you are placed on the short term planning list for services a support coordinator will be assigned to assist you or your loved one to access community resources and will continue advocating for you until funding is available. If you are placed on the long term planning list, it will be important for you to contact the Intake and Evaluation Team if your situation or needs become more critical.
