​Job Title: Support Coordinator/ Intensive Support Coordinator
Report to: Area Coordinator/ Clinical Supervisor
FLSA Status: Exempt


Job Responsibilities & Performance Standards:
  1. Communicate the mission and vision of Creative Consulting Services, Inc. to individual, families and community resources.
  2. Assure individual choice is fully offered and acknowledged while monitoring safety, health and quality of life.
  3. Collaborate with families, natural supports, providers, physicians and other community resources.
  4. Advocate for individual needs as identified.
  5. Assure completion of all required paperwork, this includes but is not limited to documenting all consumer and consumer related contacts, Serious or unusual incidents, DMA-7, ISP’s and SIS.
  6. Facilitate and Assist in obtaining and maintain Medicaid eligibility for individuals we serve.
  7. Facilitate ISP Meetings.
  8. Maintain a caseload of 40 individuals
  9. Provide support in developing community natural supports.
  10. Participate in all required training and in-service activities.
  11. Check your assigned CCS voicemail box and email regularly and respond to messages within 24 hours.
  12. Maintain a professional attitude and dress appropriately when representing Creative Consulting Services, Inc.
  13. Maintain current knowledge of all state standards, guidelines and policy manuals related to service provision for individuals with developmental disabilities as described in the CCS Policy and Procedure Manual.
  14. Possess a working knowledge of current technology and software used in the field. This includes but is not limited to Microsoft Excel, Word and basic Internet access and use.
  15. Complete all other duties as requested by the Area Coordinator and Administrative staff in response to contractual changes and requirements.
